We provide community nursing supports to you which require higher levels of care assistance. This service usually involves advanced care or training supplied by our highly trained Enrolled Nurse (EN) or Registered Nurse (RN). For example, bowel care, catheter care, wound care management.
Daily Personal Activities
We assist you with complex daily personal activities. Such as Bowel care, Urinary Catheter Care, wound care and PEG Feeding and Management.
Travel and Transportation
We assist you with travel/transportation to and from destinations.
Participation in the Community
We assist you to attend areas in the community. For example, shopping trips, appointments, leisure activities.
Group and Centre-Based Activities
We take to activities that are within a group or center based which allow the participant to have enjoyment in various activities or events.
Innovative Community Participation
We assist you to access and enjoy activities that you enjoy. This provides you encouragement to increase social interactions and participate in activities or event that you enjoy.
Daily Tasks / Shared Living
We assist you with home and personal care within a specialized disability accommodation (SDA) or within a shared living arrangement. We will be responsible for entering an SDA home and providing services including personal care, personal hygiene, showering etc.
Household Tasks
We assist you to maintain your general area of living which includes cleaning, basic indoor & outdoor home maintenance, meal preparation, gardening and laundry/linen.
Life Stage Transition
We provide assistance to boost your capacity to transition from one state to another stage in life. For example, High school to university. We provide services such as peer support, mentoring, mental health recovery, coaching, Support coordination for Level 1 (small packages) and 2 (more complex packages).
Development of Life Skills
We assist you to develop and maintain important life skills such as learning about public transport, resumes, taxes, saving skills, social skills, physical activity skills etc.
Access and Maintain Employment
We assist you to obtain and retain employment and new employment opportunities.
